VILLAGE SERVICES

Help each other out.

The Seacoast Village Project fulfills its mission of creating a community of older adults who work together to improve their odds of being able to grow older in their homes and communities by getting connected, getting smart and helping each other out.

The Village offers two types of support services to its members:

Practical Support Services
Provided by member-volunteers, the Village offers practical support services at no cost. Examples include: telephone check-ins, friendly in-person visits, referral/help with navigating/finding local medical and contractor resources, handyman tasks, yard work (planting, weeding, seasonal cleanup), outside storm preparation and recovery assistance, emergency planning, help with administrative tasks, post-hospital check-in and support, rides to outpatient hospital services, doctor appointments, and to events and errands

Health Support Team

Health Support Team provides support, information and resources to members with health challenges or who need help navigating the healthcare system. The team is comprised of Village members who are retired doctors, nurses, social workers, and other healthcare professionals, as well as those members who are caregivers and experienced healthcare consumers.

We can provide information and resources, navigate medical appointments, support transitioning between sites of care, help manage returning home from a long healthcare stay, help navigate a new diagnosis or existing condition, help when there is a flare-up or exasperation of a chronic condition, and provide support at end of life.

We are grateful to the Foundation for Seacoast Health for supporting this work!
